For any service,
please request an invoice to send to your insurer for reimbursement or
to deduct on your taxes.

appointment:

Insurance paperwork must be filled out if you need us to bill your health
insurance, L&I or workers comp, or auto accident insurance. Please note
that you also need to bring your insurance card, driver's license or
identification, and your doctor's referral and/or prescription to your
appointment.
Please note:
We ask that if you need to cancel or reschedule an
appointment, please give us at least 24 hours notice.
If you do not show for a scheduled appointment without
canceling, you will be charged the full fee for the scheduled service.
If you cancel with less than 24 hours notice, you will be charged half
the fee.
There will be a charge for any checks returned due to
Non-Sufficient Funds (NSF). Fee for returned checks is currently $25
per Washington
State law
(RCW62A-3-515).
We reserve the right to treat each situation on a case-by-case
basis. Payments for cancellations and no-shows are due upon receipt of
the invoice.
Massage Doctor is HIPAA compliant and protects your
privacy. You may be asked to sign a release for interaction with your
