Fill this out if you need us to bill your health insurance, L&I or workers comp, or auto accident insurance. Please note that you also need to bring your insurance card, driver's license or identification, and your doctor's referral and/or prescription to your appointment.

If you are a under age 18, it is Washington State law that a parent or guardian consent to treatment for massage therapy -- please also print your Parental Consent.
