For any service,
please request an invoice to send to your insurer for reimbursement or
to deduct on your taxes.
Before your
appointment:
First Time Client? Please
print your Health
Intake Form. This will give
you the opportunity to fill it out before your appointment and save
some time.
For billing insurance, also print your Insurance Form. (Fill this out if you need us to bill your
health insurance, L&I or workers compensation, personal injury, or
auto accident insurance)
Please note:
We ask that if you need to cancel or reschedule an
appointment, please give us at least 24 hours notice.
If you do not show for a scheduled appointment without
canceling, you will be charged the full fee for the scheduled service.
If you cancel with less than 24 hours notice, you will be charged half
the fee.
There will be a charge for any checks returned due to
Non-Sufficient Funds (NSF). Fee for returned checks is currently $25
per Washington
State law
(RCW62A-3-515).
We reserve the right to treat each situation on a case-by-case
basis. Payments for cancellations and no-shows are due upon receipt of
the invoice.
